Letter of Agency

A signed letter of Agency is required if you are porting your phone number. By filling out this form you are authorizing Long Lines to transfer your phone number from your current provider. We will also need a copy of your most recent telephone bill with the account number and billing address.

Typing your name serves as an "electronic signature" and allows us to process this part of your order. Thank you.
